3.	MM2H Application Form
4.	Four (4) passport-size colour photographs for each of the family member
5.	Three (3) copies of Form IM.12 form for each family member
6.	Certified copies of all pages of Passport/travel documents - Applicable for all family members
7.	Personal Bond Must be completed and signed by Malaysian who is one of the Board of Directors/ Shareholders/ Managing Director/ Manager from the appointed MM2H licensed company
8.	Self Declaration on dependants' health condition
9.	Certified copy(s) of latest 3 months' bank statement or related financial documents so as to indicate the financial capability to support stay in Malaysia
10.	Latest 3 months' certified copies of pay slip/ income statement (If employed)/ pension slip
11.	Certified Copy of Marriage Certificate (if accompanied by spouse)
12.	Certified Copy of Birth Certificate (if accompanied by children under 18 years old) Letter of confirmation from Medical Specialist/ General Practitioner (if accompanied by disabled children above 21yrs old) Statutory declaration by principal applicant to bear all expenses and financial requirements for dependants during their stay in Malaysia
13.	Authorization letter from applicant to Malaysia My Second Home Centre for verification of financial documents with the relevant financial institutions
